Why did I receive a bill/EOB from Lux Dx? I do not remember having any procedures performed.
A bill/EOB from LUX DX is likely from clinical lab services your doctor ordered. If you have had urinalysis, cultures, or blood drawn at your doctor’s office, or were a patient at one of the many Skilled Nursing Facilities we collect samples and perform clinical pathology services, you may be receiving a statement for clinical laboratory services performed at Lux DX.
Why didn’t insurance pay for this test?
Your insurance carrier should send you an Explanation of Benefits (EOB) that explains in detail the services that were either paid or denied. You may also find this information on the invoice. If you need additional information, you should contact your insurance carrier directly to determine the reason(s) why your insurance company did not pay your bill.
